visit this area of the site to fill out your intake forms.
We have two options to complete these forms:
- Fill
them out online via our secure form system
- Download the form files (in PDF format), print
them, and bring them to your first appointment
A few common questions about our online
intake form system:
Can anyone view my information besides Dr. Higginson?
Dr. Higginson may designate an authorized employee to view your records. Otherwise,
your information is protected by the Health Information Privacy Act (HIPAA),
and it may not be viewed by anyone else without your express consent.
Is it possible that anyone else can view these forms after
I fill them out?
These forms are protected with the same encryption used to guard against intruders
into commercial retail web sites. We establish a standard secure internet connection
between your computer and our server. This connection uses 128-bit encryption
to protect your information as you transmit it to us.
