Steps to download forms:

1. Click on one of the two links above, depending on whether you have recently been in an accident or not
2. The pdf should open in your browser or in a separate application. If it does not, locate it in your downloads folder and double-click it
3. Print the forms
4. Fill them out by hand
5. Bring them to your first appointment with Dr. Bingham
